Effectiveness of glucose-lowering drugs is the focus of the multi-year research project

WASHINGTON - John Lachin, professor of biostatistics, epidemiology and statistics at the George Washington University, has been awarded a five-year, $134 million grant from the National Institute of Health's National Institute of Diabetes and Digestive and Kidney Diseases to conduct a clinical trial examining the long-term effectiveness of several glucose-lowering medications for treatment of people with type 2 diabetes. The grant sets a record as the largest sum award GW has ever received.

Dr. Lachin, interim director of GW's Biostatistics Center in Rockville, Md., is principal investigator for the grant along with David M. Nathan, M.D., director of the Diabetes Center at the Massachusetts General Hospital and professor of medicine at Harvard Medical School, both in Boston. The pair, working jointly with investigators at GW and Massachusetts General Hospital, recruited a network of more than 40 clinical centers nationwide to participate in the study.

"This funding is tremendously important to healthcare as it could result in more effective treatment of diabetes," said Leo Chalupa, vice president of research at GW. "The award of this grant signifies that the Biostatistics Center is a premier facility of its type and that Dr. Lachin and the project team deserve tremendous credit for putting together this successful investigator-initiated grant application. An award of this magnitude will increase the future ranking of GW among the country's research universities and bring us closer to the goal set by President Knapp of becoming a top tier research institution."

Titled Glycemia Reduction Approaches in Diabetes: A Comparative Effectiveness (GRADE) Study (grant # 1 U01 DK-098246), the clinical trial will directly compare the most commonly used medications to treat type 2 diabetes.

"Type 2 diabetes is an epidemic that threatens to become the century's major public health problem and poses enormous human and economic challenges in the U.S. and worldwide," said Dr. Nathan.

Dr. Nathan is the senior medical and scientific investigator for the project and chairs the GRADE Research Group. The director of the Coordinating Center, Dr. Lachin, is the senior biostatistician for the project and is responsible for the coordination of all study research activities. Together they will provide the scientific leadership for the project and oversee all study activities.

The study will address the challenges of type 2 diabetes in a randomized, clinical trial of 6,000 patients with recent onset of type 2 diabetes. The GRADE study will compare the effects of four glucose-lowering medications - the sulfonylurea glimepiride, DPP-4 inhibitor sitagliptin GLP-1 agonist liraglutide and basal insulin glargine, added to metformin over about four years and track their success in combating the disease. Each of the drugs will be used in combination with metformin, the widely accepted first line medication for treatment of type 2 diabetes.

"For people with type 2 diabetes, this is a compelling need because numerous competing treatments are available," said Dr. Lachin. "While each agent was found to safely and effectively reduce blood glucose for relatively short periods such as six months to a year, type 2 diabetes will usually require continued treatment over a lifetime. GRADE'S primary objective is a direct comparison of the commonly used drugs over a more realistic period of time with regard to glucose-lowering, side-effects, tolerability, other effects and costs."

Glucose management is important in diabetes patients because sustained elevated levels of blood glucose can lead to extensive damage to the circulatory system which then can lead to a host of diabetic complications. Damage to the nerves can lead to amputations of the extremities, particularly feet; damage to the kidneys can lead to end stage renal disease requiring dialysis or transplantation; and damage to the eyes can lead to impaired vision or blindness.

"The goal of therapy should be to maintain a blood glucose level as close to the non-diabetes range as possible to prevent or slow the progression of these complications," said Dr. Lachin. "The principal study objective is to determine which of these therapies most effectively and safely reaches and maintains such near normal levels of blood glucose."

More than 40 participating clinical centers each plan to recruit at least 150 eligible patients who have consented to be randomly assigned to one of the four therapies under study. Funds from the grant to GW will be dispersed to these centers to support their activities in the GRADE study.

GW's Biostatistics Center will coordinate all research and operational activities at all sites for the study.

The initial steps of the project are to begin Oct. 1. The present grant supports the first five years of the project, with the opportunity for renewal for an additional three years. The total project duration is scheduled to be eight years.

The Ultimate House Party with Ed Sheeran! | The Student Pocket ...

What a night! The three words which instantly spring to mind when looking back at what truly was ?The Ultimate House Party?. After many months of planning and many weeks of waiting, the special night finally took place this summer, which for one person in particular, was a night that will never be forgotten. Ed Sheeran, double Brit Award winner, with a platinum selling debut album, countless singles in the charts and an overall popularity which can?t be put into words, made the dreams of one north London student come true this May; performing an exclusive secret garden set in the presence of her closest friends and family.

It may well be the case that you had already heard of this amazing prize we at The Student Pocket Guide were giving away. Way back in the summer of 2011, just before the meteoric rise of Ed Sheeran had begun, we were lucky enough to catch up with him on his UK tour to arrange a competition that couldn?t be matched: a one off house gig, with provided drinks and cleaners the next day. What followed was a influx of interest and popularity with what was on offer. The eyes of the nation?s newspapers were caught, the voices of blogs and websites were heard and competition entries soon tipped the hundreds and then thousands, out of which, one person would be chosen, for one very special night. Hannah Goodwin, 21 years old of Hoddesdon, Hertfordshire, was this lucky person who played host to this once in a lifetime occasion. Here is how it went!

Simply put, it couldn?t have gone any better. Great people, great setting and of course, an unforgettable performance by Ed Sheeran. The months of hard work came to fruition with everything coming together perfectly; even the sun came out to catch a glimpse of the nation?s favourite red head!

Many of Hannah?s friends had no idea what was going on when we first showed up at her house. ?Why are there cameras here??, a few of them whispered to each other. And when Ed casually walked through the front door, then out into the garden, you can imagine the utter shock splattered on their faces! After mingling with the crowd and greeting Hannah and her family, Ed exchanged a few words with us sat on a bench at the back of the garden, in front of an eagerly listening crowd. ?You know what??, he begins,? ?I?ve done so many back garden gigs. It was actually how I used to make a living; going around student houses with a bag of CDs making ?100 a time.?

We went on to discuss his recent time spent in America on tour with Snow Patrol; a tour which he unfortunately had to cut short by a few shows in May. That aside, Ed went on to explain how things over there have compared to his journey in the UK. ?It?s very similar to how things were last May in Britain. We?ve started creating a small buzz over there, so hopefully it builds.? His focus since has shifted back to the UK, having played a series of shows and festivals. Many of you Ed Sheeran fans I am sure would have seen his brilliant performance at BBC Radio One?s Hackney Weekend on TV, or, if you were lucky enough ? at the show, which was a highlight it seems for so many artists this summer. There is always one venue or festival, however, that is a particular favourite that lives long in the memory for many artists. For Ed, this was Reading 2011. ?It was mental. I played the Festival Republic tent and it was so good. Very, very fun!?

As the conversation went on, we spoke about Ed?s love of cucumber (randomly), future records he?s been working on and Lego building amongst a host of other things, including the possibility of a headline arena tour. ?Arenas are difficult to do because you have to hold a crowd for one and a half to two hours; and I don?t have a band so I?d be doing it on my own. As soon as I have enough songs to be able to do a good set, I?ll play an arena tour!? One thing we didn?t have to wait to see was his latest tattoo! As he showed us and everyone there, he explained the story behind his latest addition. ?I grew up next to a cafe called ?The Dancing Goat?, which my best friends own. I walked to school with them every day for like seven years, so I thought I?d get a goat!? We spoke a little more then closed the interview to a rapturous applause, which signified the moment that everyone had been waiting for, Ed?s intimate set.

The garden was lit with outdoor lamps and candles, which coupled with the sunset, just as the gig was in full flow, created a perfect serene atmosphere. Out came the classics of ?Lego House?, ?The A Team?, and ?Small Bump?, with all in attendance singing along and getting quite simply blown away. Even the mosquito buzzing around the sound recorder was having a blast! The ?My my, my my? of ?Give Me Love? resonated around the garden, with everyone smiling as they repeated Ed?s famous lyrics. You could sense everyone there trying to capture the moment; realising that this really was a once in a lifetime occasion.

Then came the highlight of the night. ?You Need Me, I Don?t Need You?, which even for Ed, was something special. As Ed began the second single from his debut album ?+?, in the corner of his eye to his amazement, was Hannah?s brother, Niall, rapping and singing every word. Even the camera crew had to remove their headphones to believe what they were hearing!? The rest of the song soon became a word-for-word duet, with both guys going at it full flow, rapping lyrics, ?I?m always doing shows if I?m not I?m in the studio; truly broke, never growing up call me Rufio; melody music maker, reading all the papers; they say I?m up-and-coming like I?m? (You know the rest!)?, which received the biggest cheer of the night.

It was the perfect end to a mesmerising set. The final chord was met with a great cheer, and as Ed stepped down from the top of the garden, he left a crowd ecstatic from what they had just experienced. ?I?m kinda lost for words. I can?t describe how happy I am!?, Hannah told us afterwards. Her brother, Niall, was also left a little shell shocked. ?I don?t realise what I?ve just done, I?ve just sung with a guy who has two Brit Awards! Amazing?, he told us with a look of disbelief at what just happened. Everyone congregated in the garden with drinks and food, still buzzing about with a childish excitement. Photos were taken with Ed; CDs, arms, legs and shoes were signed, and all attending had their final chance to have a few words with the man of the moment, rounding off an unforgettable night.

Seeing an artist of Ed?s calibre in an environment of such intimacy was a real privilege. It was an experience that we will never forget and one that we are truly thankful for. Obesity-related hormone discovered in fruit flies

ScienceDaily (Sep. 27, 2012) ? Researchers have discovered in fruit flies a key metabolic hormone thought to be the exclusive property of vertebrates. The hormone, leptin, is a nutrient sensor, regulating energy intake and output and ultimately controlling appetite. As such, it is of keen interest to researchers investigating obesity and diabetes on the molecular level. But until now, complex mammals such as mice have been the only models for investigating the mechanisms of this critical hormone. These new findings suggest that fruit flies can provide significant insights into the molecular underpinnings of fat sensing.

"Leptin is very complex," said Akhila Rajan, first author on the paper and a postdoctoral researcher in the lab of Norbert Perrimon, James Stillman Professor of Developmental Biology at Harvard Medical School. "These types of hormones acquire more and more complex function as they evolve. Here in the fly we're seeing leptin in its most likely primitive form."

These findings will be published September 28 in Cell.

In order for an organism to function normally under varying conditions, its organ systems must learn to maintain a steady state, or "homeostasis." Coordinating food intake and nutrient stores with energy requirements is a key homeostatic mechanism referred to as energy homeostasis. Leptin regulates energy homeostasis by linking the organism's fat stores with caloric intake. It is the hormone that tells the brain, "You've had enough."

Researchers have known for the better part of a decade that molecules secreted by the fruit fly's fat tissue communicate such nutrition status reports throughout the fly's entire body. However, they have not known the identity of these molecules, or the nature of the signals they transmit. Rajan hypothesized that this signaling molecule most likely resembles the leptin hormone in humans, since flies and mammals share similar nutrient-sensing pathways.

Researchers had predicted that three molecules in flies were likely to be structurally similar to leptin. When Rajan knocked out one of them, a protein called Upd2, the flies behaved, on a metabolic level, as though they were starving -- despite consuming their normal caloric content.

"Since leptin is a nutrient sensor, this makes sense," said Rajan. "If you knock out the molecule that senses nutrients, the body thinks there are no nutrients. Blocking this molecule copied the phenotype of starvation."

Further tests showed that when flies were actually starving, levels of Upd2 went down, and when they received adequate nutrition, levels went up. This provided further evidence that, like leptin, Upd2 is a nutrient sensor.

Next, the researchers found that Upd2 uses a neural circuit similar to that of leptin to traffic nutrition information between the brain and fat tissue. When Upd2 reaches the brain, it regulates insulin secretion, in effect "telling" the fly to store nutrition and expend energy on growth.

Finally, Rajan and colleagues engineered a fly that lacked Upd2 altogether and inserted the human leptin gene in its place. The fly fully incorporated this mammalian molecule, and all normal nutrient-sensing functions resumed.

"The key significance here is that we can now take full advantage of the sophisticated genetic tool kit available in fly genetics to address profoundly complex questions pertaining to leptin biology," said Perrimon. "This is good news to scientists studying obesity at the molecular level."

Interestingly, the amino acid sequence of leptin diverges from that of Upd2. However, the proteins produced by each gene share many structural similarities. "There are very few examples of this in the literature," Perrimon said.

"Now that we've identified Upd2 as a fly's nutrient sensor and have begun to work out the brain circuitry, the next step is to go deep into the mechanisms," added Rajan.

Positive Review of '2016: Obama's America' from an Obama Voter

I reviewed the excellent?Dinesh D?Souza movie??2012: Obama?s America?the other day on this blog. Raking in about $26 million so far, the film is the second-highest-grossing political documentary of all time, and the sixth-highest-grossing documentary period.?Word is the film will be out on DVD and pay-per-view before the election.

I came across a review of the movie from an unusual source: a man who voted for Obama in 2008 and had his eyes opened after viewing the documentary. Read his whole review here. I?ll share a few excerpts:

I had the displeasure of seeing 2016 the movie yesterday. But not in the way you may think. You would think that as a former Obama fan, it would have been a right-wing propaganda filled experience. A waste of my time. It wasn?t! ?

I was quite honestly amazed at the many things I learned ? right from the horse?s mouth, so to speak? from his brother, and others who knew him ? The movie was very well done and very factual. It was scarier than any Friday the 13th movie I have ever seen. ?

There was not a sound in the theater as the movie progressed and when it ended, people just sat there, stunned. There was not a nay-sayer or heckler in the entire group. I might have been one if them had I not been so shaken.

It really is a revealing look of who Obama is and where he and his handlers want to take our beloved country, now that the waters have been tested as to what he can get away with, what it reveals his plans for the next 4 years and beyond is, well, reprehensible. I voted for this man, I had his best wishes in my heart, but he has not returned the favor to the American people. In fact, he seems completely unaware of owing Americans anything, blacks, whites, Hispanics ? they are merely a means to an end: a vote to further his agenda would end our America, the way we have known it.

No matter your political leanings, if you love America, you owe it to yourself to see the movie.

What we didn't get on the iPhone 5 - Know Your Cell











Apple officially introduced the iPhone 5 earlier this week and it's looking like it will be a massive sales hit. Apple has brought a lot to the table with its new smartphone, but it is still missing a few things that the competition can boast about.?

NFC

Many thought that the iPhone 5 could really be the catalyst for mobile payments, but the lack of an NFC chip means that we'll probably continue to face fragmentation in this market.?

Apple said it doesn't think NFC solves a real problem?for consumers today and it believes that its upcoming Passbook service could fill a lot of the mobile payment or mobile boarding pass needs. I think that's true to a certain extent, but I really believe Apple could have kicked mobile payments into gear by including NFC.?

Services like ISIS?and Google Wallet rely on NFC but neither has gained much traction. ISIS is delayed and Google Wallet has still failed to break out in the mainstream. You could see why Apple doesn't want to boost Google Wallet, but ISIS could have been a success if the iPhone 5 was its flagship device.?

NFC is relatively cheap to put in a phone too, and we now expect it out of every major smartphone. The metal backing of the iPhone 5 may provide design difficulties, but I think Apple could have gotten around it if they really wanted to.?

Wireless Charging

The Nokia Lumia 920?looks like a really cool smartphone and one of the standout features is that it will have the ability to do wireless charging. Sure, it's not revolutionary but it's definitely something that's nice to have.?

The iPhone 5 will charge by plugging it in to a Lightning port. Apple said?it didn't include wireless charging because wireless charging solutions still have to be plugged into a wall anyway, so users aren't really saving much time or effort.?

That's also kind of true but I've found wireless charging pads and devices can be pretty neat. Sure, plugging in a cable isn't a huge hassle but every little convenience adds up. The Lumia 920 uses the Qi wireless charging standard too, so it's not like it would have been insanely difficult for Apple to include this.?

Wi-Fi Direct

Wi-Fi Direct is another standard that Apple decided not to include even though it may not have been that difficult from a technical standpoint. Wi-Fi Direct is in a lot of high-end smartphones nowadays and it enables users to send data quickly to other devices without the need for a Wi-Fi network. It's the height of convenience.?

We had heard rumors that Apple was going to be expanding its AirPlay streaming capabilities into something called AirPlay Direct. This would let users stream content from their iDevice to specially certified devices without the need for Wi-Fi. So, basically, it's Apple's own version of Wi-Fi Direct.?

We didn't see it in the iPhone 5 though and that's a shame.?

New colors

This one is very superficial but why can't we get an iPhone 5 in more than just black and white? I'm not saying these aren't attractive or stylish but I think a splash of color would be much appreciated.?

The Lumia 920?comes in a splashy yellow, the Galaxy S3 comes in an eye-catching red and various other smartphones let you personalize your device by choosing a non-standard color. I suppose you can express yourself on the iPhone 5 with a funky case but I still think Apple should offer a few more colors.?

Stock futures edge lower ahead of Fed decision

NEW YORK (Reuters) - Stock futures slipped on Thursday as investors awaited data on the labor market and a possible decision on further economic stimulus measures at the conclusion of a policy meeting by the Federal Open Market Committee.

The Federal Reserve appears poised to launch a third round of unconventional monetary stimulus, while signaling that a weak economy may warrant ultra-low interest rates for at least another three years. The FOMC announces its decision at about 12:30 p.m. EDT at the close of a two-day meeting.

With the S&P 500 <.spx><.inx> up more than 9 percent since the start of June on expectations global central banks will act to combat slowing growth, some analysts are wondering how much further the index can climb.

Wall Street ended little changed on Wednesday, erasing early gains in cautious trading.

"Yesterday's action (in U.S. equities) was pretty indicative that everybody is waiting to see to see what action the Fed is going to take: Is there going to be a change in language, is there going to be no change or is there going to be the announcement of a new program," said Kim Forrest, senior equity research analyst, Fort Pitt Capital Group in Pittsburgh.

"If they only do a language change or don't take action, the traders who have been the drivers of the market on the way up may drive the market downward."

At 8:30 a.m. EDT (1230 GMT), the Labor Department will release weekly initial jobless claims data. Economists in a Reuters survey forecast a total of 370,000 new filings compared with 365,000 in the prior week.

The August producer price index will also be released at 8:30 a.m. EDT (1230 GMT), with expectations for a 1.1 percent rise compared with a 0.3 percent increase in July. Excluding volatile food and energy items, PPI is expected to rise 0.2 percent, against a 0.4 percent increase in July.

S&P 500 futures fell 2.2 points and were slightly above fair value, a formula that evaluates pricing by taking into account interest rates, dividends and time to expiration on the contract. Dow Jones industrial average futures lost 25 points, and Nasdaq 100 futures shed 4.5 points.

European shares dipped, edging back further from the 14-month high hit in the previous session, with some saying action from the U.S. Federal Reserve is needed to provide further support to prices. <.eu/>

Britain's BAE Systems and Airbus-owner EADS said they are in advanced talks to create an industry giant that would overtake rival Boeing in sales and contend with defense cutbacks in Europe and the United States.

Asian shares slipped slightly in choppy trade ahead of the Fed decision, with investors cautiously optimistic of further stimulus action to bolster the world's largest economy.

Pagoda Box Is Easier Than Amazon Web Services, But More Customizable Than Heroku

pagodabox_logoI've had a sense for a while that infrastructure-as-a-service (like Amazon Web Services) and platform-as-a-service (like Heroku) are converging. Developers love the idea of using a PaaS to speed up provisioning and deployment, but don't necessarily want to completely give up control of their environments. One sign of this convergence is Pagoda Box, a PaaS that provides a deeper level of control over infrastructure while still abstracting away all the hard stuff.

Apple details iPhone 5's new camera 8MP, 'same as iPhone 4S but thinner'

Apple's got a new camera its brand new iPhone 5: 8-megapixels with a 3,264 x 2,448 backside-illuminated sensor, five-element lens, and f/2.4 aperture sits on the rear of the new device. "Same as the iPhone 4S, but thinner." 25 percent thinner in fact, to match the now slimmer iPhone. The branding remains the same -- it's still the iSight -- but a few new bells and whistles have been added alongside the slimming. A new dynamic low light mode, for one, and a fancy sapphire crystal cover. The phone's new A6 processor also helps out, offering a "smart filter" for better color matching, as well as speedier capture and still capture during video.

In terms of functionality, there's a new panorama mode, that allows picture stitching for multiple images into one larger shot (a whopping 28 megapixels in total). Photo sharing is also getting expanded, adding more social functionality -- images can be shared with multiple friends or family members, and then those folks can comment.

But you only care about video, don't you? You'll be glad to hear that it's getting expanded as well. The rear camera is identical to the 4S, albeit with better image stabilization. The front facing camera now offers 720p video capture for all your extreme close-ups -- also for FaceTime, which will be available via LTE on the iPhone 5. But on all cellular networks? We're not so sure just yet.

Since Medicare's creation in 1965, the eligibility age has been 65 (for people without disabilities). Some proposals would gradually raise Medicare's eligibility age from 65 to 67.

iPhone 5 using new Lightning dock connector

The iPhone 5 announced today at Apple's big event will include a new dock connector called Lightning. We saw it often enough in the pre-show leaks, and is now fully official. It includes a new all-digital 8-signal design, has an adaptive interface, improved durability, reversible (i.e. usable from both ends), and is 80% smaller than the old dock connector.

As for all of those accessory manufacturers bashing their head against the table now, Apple will be providing an adapter which will allow old docks to plug into the new Lightning connector. That said, there are already a lot of big names already well on their way to making Lightning-compatible accessories, including Bose, JBL, B&W, and B&O. Whether or not this new cable is any faster or useful than the old one is anybody's guess, but at least it's smaller, and at this point, Apple really needs to save as much room as possible in their devices. In any case, let's pour one out for our old dock connector. It's been a good 9 years.

Gomez's goal gives US revenge vs. Jamaica

Days after stunning loss in World Cup qualifying, team earns 1-0 victory

COLUMBUS, Ohio - A curling free kick from Herculez Gomez gave the United States a much-needed 1-0 win over Jamaica in their CONCACAF region World Cup qualifier in front of a passionate crowd in Columbus, Ohio, on Tuesday.

The U.S., who suffered a 2-1 defeat to the Jamaicans in Kingston on Friday, are now tied on seven points at the top of group A with Guatemala and Jamaica with two games remaining in the phase.

With only the top two from the group progressing to the final round of qualifying, the U.S. still have plenty of work to do in October's games away to Antigua and Barbuda and at home to Guatemala.

But their task would have been much tougher had they not taken the maximum points from Tuesday's game.

U.S. coach Juergen Klinsmann made five changes from Friday's defeated line-up and brought in some much-needed width with Graham Zusi on the right and Jose Torres on the left while Danny Williams was given the main defensive midfield responsibility.

The switch worked well with the U.S. dominating possession heavily in the opening 45 minutes and only the woodwork and great keeping from Dwayne Miller leaving the visitors on level terms.

Zusi made an instant impact hitting the bar with a shot from the edge of the area in the sixth minute and then 14 minutes later Steve Cherundolo saw his low shot deflected, forcing a great save from Miller who tipped the ball against the post.

Miller was again down close to foil Zusi at the back post as the U.S. camped inside Jamaica's half but he was just a bystander as a thundering strike from Williams crashed against the post.

The breakthrough finally came 10 minutes after the interval when Gomez curled a free kick inside Miller's left-hand post and while the keeper should have done better, few could argue that the Americans deserved their lead.

With the advantage in place and the Caribbean side showing more attacking intent as they searched for an equalizer, the U.S. struggled to create more chances but it was a night that was all about picking up three crucial points on the road to Rio.

"It was important for us to regroup after what happened in Jamaica, we knew it wasn't our best performance," said Gomez.

"It wasn't always pretty but I think we did the job well," added the striker.
